The problem

Every page looks important. Few of them are.

A file arrives with 900 pages of hospital records, office notes, imaging reports, and medication lists. Every page looks important. Someone in your office has to read it, put it in order, and work out what matters — and that person is usually not a clinician.

A page-by-page abstract will tell you what each note says. It will not tell you when the patient's condition changed, what the record is missing, or which finding the defense will build its case around.

What makes this different

Read the record the way a clinician does.

Every summary is prepared by Michelle Carroll, Founder and Chief Clinical Strategist of Lexcura Summit, using the same method she teaches in the Clinical Intelligence book series. She has more than four decades of clinical experience, including intensive care, emergency care, hospice, home health, and long-term care.

You receive more than a list of dates. You receive a summary that reads the record the way a clinician does:

  • Who the patient was before the event
  • When the clinical picture changed
  • What the record shows, what it suggests, and what it cannot prove
  • What is missing, and where to get it

FAQ

Questions attorneys ask.

What types of cases do you summarize?+

Medical malpractice, personal injury, wrongful death, nursing home and long-term care, home health and hospice, and other cases where the medical record decides the outcome.

Is this an expert opinion?+

No. A medical summary organizes and explains the records. It does not give a standard of care or causation opinion, and it is not expert testimony. If you need those opinions, the summary helps you choose and brief the right expert.

How are the records kept secure?+

Records are uploaded through an encrypted portal.

What if more records arrive later?+

Send them through the same portal. They are added to the chronology at the supplemental page rate.

Do you work with defense firms?+

Yes. The method is the same for both sides: it shows what the record supports and where it is weak.

How do I pay?+

Payment is made in full by card at the time of order. Work begins once payment and the complete records are received. Custom quotes, rush fees, and supplemental pages are also paid in advance.
